*Every hair extension service is customized & tailored to your wants/needs and day to day lifestyle, therefore every detail matters! Please be as specific as possible.

*MKA Beauty is a Bellami Hair exclusive salon. Miss Kayla handles all hair transactions and does not accept used or previously owned hair that hasn't been installed by Miss Kayla.

*After the form is completed, we can move forward with scheduling an in person consultation. A $50 nonrefundable deposit is required for consultations. If you choose to move forward with your extension this fee will go towards your extension service.
